Rising Heart Attack Rates During Pregnancy Demand Urgent Attention
Pregnancy, traditionally associated with robust health, is increasingly linked to heart attacks among expectant mothers. A recent analysis highlights that the incidence of these serious cardiac events during pregnancy is rising, reversing decades of perceptions that such occurrences were rare. In 2023, the maternal mortality rate in India reached 88 deaths per 100,000 live births, translating to approximately 22,500 maternal deaths for the year, with cardiac issues contributing significantly to this troubling statistic.
Understanding the Shift in Heart Health Risks
The rise in heart attacks during pregnancy reflects broader changes in maternal demographics and health. Delayed pregnancies, particularly among women aged 35 and older, are increasing, coinciding with a natural escalation in cardiovascular risk. Coupled with this trend, higher rates of diabetes, obesity, hypertension, and sedentary lifestyles have intensified risk factors among pregnant women.
Moreover, advancements in reproductive technologies, such as in vitro fertilization (IVF), often involve hormonal treatments that can complicate cardiovascular health. Conditions like spontaneous coronary dissection, though rare, are more prevalent in pregnant women and can lead to acute myocardial infarction.
Pregnancy itself places considerable strain on the heart, increasing blood volume by nearly 40%. The heart must work harder to meet the demands of the developing fetus. This physiological change, combined with pregnancy’s natural tendency to enhance blood clotting to reduce bleeding during childbirth, can complicate cardiovascular health.
Challenges in Diagnosis and Treatment
Symptoms of heart attacks during pregnancy often diverge from classic presentations, leading to potential delays in diagnosis. Patients may experience breathlessness, nausea, severe fatigue, or discomfort in the back or abdomen rather than the typical severe chest pain. Such atypical symptoms can mislead both patients and healthcare providers, resulting in a dangerous false sense of security.
Timely intervention is crucial, as restricted blood flow to the heart can have severe consequences for both mother and fetus. Treatment protocols require a coordinated approach, involving cardiologists, obstetricians, anaesthetists, and neonatologists. Unfortunately, access to such specialized care can be inconsistent, particularly in smaller medical facilities, exacerbating referral delays and increasing risks.
Preventive measures and early detection are essential for improving outcomes. Women at risk should undergo comprehensive cardiovascular evaluations prior to or early in their pregnancies. Regular monitoring of blood pressure and blood sugar levels, coupled with a healthy diet and physical activity, can significantly mitigate risks.
As global data indicates a rising trend in maternal deaths linked to heart disease, it is crucial for women to recognize that pregnancy does not inherently protect heart health. Symptoms such as chest pain, unexplained fainting, or sudden changes in well-being should prompt immediate medical evaluation.
While myocardial infarctions during pregnancy remain infrequent, their impact can be devastating. Proactive detection of risks and timely interventions can ensure that pregnancies proceed without complications, allowing for healthier outcomes for both mothers and infants.

Manokar, a senior interventional cardiologist and clinical lead of the heart failure and transplant program at Kauvery Hospital, raising awareness about these issues is essential for safeguarding maternal health. Timely and effective care can transform the pregnancy experience, making it joyful and complication-free.
