Doctor Uncovers Rare Disease in 42-Year-Old Engineer
A 42-year-old engineer, identified only as MK, received a life-changing diagnosis thanks to the keen observations of Dr. Lata Bichile, a consultant rheumatologist at the Centre for Arthritic and Rheumatic Diseases Treatment in Mumbai. MK had been suffering from debilitating pain and stiffness, particularly in his right forearm, for four months before seeking medical help in August 2023.
Upon examination, MK reported that simple tasks had become increasingly painful. He described the sensation as if “something inside is tightening.” This alarming symptom led to further investigation, as the pain worsened and spread to his thighs and left upper arm, making everyday movements a challenge.
Dr. Bichile noted that while MK’s hands and feet appeared normal, the skin over his forearm and thighs showed slight puckering, suggesting an underlying issue. Despite these visible signs, key indicators such as fever, weight loss, or respiratory issues were absent, complicating the diagnosis.
Identifying a Rare Condition
Dr. Bichile suspected a rare inflammatory disease known as Eosinophilic Fasciitis, also referred to as Shulman syndrome. Since its identification in 1974, only a few hundred cases have been reported globally, making it difficult to recognize.
Blood tests revealed elevated levels of inflammation markers, specifically ESR and CRP, alongside significantly high levels of eosinophils, a type of white blood cell associated with inflammation. Normal muscle enzyme levels helped rule out other muscle diseases, while autoimmune tests returned negative results, excluding conditions such as systemic sclerosis.
To confirm the diagnosis, an MRI was performed on MK’s thighs. The imaging results displayed inflammation and fluid accumulation along the fascial layers, a hallmark of Eosinophilic Fasciitis, with no damage to the muscles themselves. Dr. Bichile expressed relief that the condition had been identified early, stating, “It was rare but treatable.”
Effective Treatment and Recovery
Following the diagnosis, MK commenced treatment with high-dose steroids, tailored to his body weight, along with a steroid-sparing medication to ensure long-term safety. Physiotherapy was integrated into his recovery plan to facilitate mobility and strength.
Three months after starting treatment, MK experienced significant improvements. His elbow movement returned to normal, thigh stiffness diminished, and shoulder pain subsided. The reduction in skin tightening allowed him to resume work without interruption. Dr. Bichile praised his commitment to treatment and the trust he placed in the medical process.
In her report, Dr. Bichile emphasized the importance of awareness surrounding rare diseases. With nearly 35 years of medical experience, she has encountered only two cases of Eosinophilic Fasciitis. She remarked, “Every rare patient reinforces the importance of awareness, early diagnosis, and trust in treatment.”
This case serves as a reminder that even uncommon inflammatory diseases can present as typical aches and pains. Sometimes, successful outcomes in medicine manifest in simple, everyday activities, highlighting the profound impact of timely diagnosis and effective treatment.
